Ischemic testicular torsion: a case report

Abstract

Introduction: testicular torsion is considered a urological emergency, it occurs when one of the testicles rotates around its own axis, culminating in torsion of the spermatic cord and compromising blood flow to the region and consequent ischemia. Early diagnosis and prompt surgical intervention are crucial in order to prevent the manifestation of irreversible ischemic lesions, as delays in treatment can trigger a reduction in fertility and the indispensability of orchiectomy. Objective: to analyze the clinical management of acute testicular torsion, emphasizing the importance of early diagnosis and immediate surgical intervention. Method: descriptive-analytical case report, featuring a patient selected based on clinical, radiological, and surgical criteria indicative of acute pain in the right testicle, requiring immediate assessment and treatment. Results: the acute manifestation of intense pain in the testicle is an alarming sign of testicular torsion. Ultrasound of the scrotum plays a fundamental role in confirming the diagnosis, indicating vascular involvement with color Doppler, strengthening the imperative of immediate surgical exploration. Given the confirmation of necrosis of the testicular tissue, it was decided to perform a total orchiectomy on the right testicle, with the aim of preventing future complications, as well as fixing the contralateral testicle in the same procedure, aiming to mitigate the possibility of torsion on that side. Conclusion: testicular torsion remains a urological emergency that demands rapid attention and intervention. The crucial nature of rapid diagnosis and prompt surgical approach in preserving testicular health was highlighted. The patient's clinical condition, based on confirmed tissue necrosis, led to total orchiectomy, aiming to safeguard his health.
